To schedule an appointment, please call our appointment desk at 462.6200.  If you need to cancel an appointment, please call the office at least 24 hours in advance.

Clinic Information


The patient portal may be used to register as a new patient and access other clinic forms that may be needed for your visit.  If you choose to fill your forms out by hand, please arrive a few minutes early to check in for your appointment.  New patients will be asked to fill out a health history questionnaire and should arrive 20 to 30 minutes early.  Be prepared to present your identification and insurance card when you check in.


Our goal is to treat urgent medical problems within 24 hours.  If you need immediate medical attention during regular office hours, please call our office.  Some appointments are set aside for emergencies.  

If you have an urgent need after hours, please call our office.  The answering service will take your name and phone number and contact the doctor on call.  

Please remember that your insurance may require a doctor’s referral for emergency care which in not life-threatening.

Immediate Care

Payments (or co-payments) are required at the time of service.  We accept Visa and Matercard.  

We accept most insurance plans.  For your convenience, we will file your claims for you.  Please bring your insurance card when you visit the office so we can file your claim promptly.

Let us know if your injury is related to a motor vehicle accident.  Auto insurance and your traditional medical insurance information will be required.
