What is a PCMH?
A Patient Centered Medical Home (PCMH) is not a building, house or hospital, but rather an approach to providing comprehensive health care. A PCMH is called a “home” because we would like this office to be the first place you think of for all your medical needs. Our goal is to make it easy and comfortable to get the care you need in a way that works best for you. As your healthcare providers, we are here to facilitate a personal partnership with you and your family to provide you with the best quality, comprehensive and progressive primary care.
Appointments: To schedule an appointment, please call our appointment desk at 316.462.6200. If yo need to cancel an appointment, please call the office at least 24 hours in advance.
Registering: New patient forms may be accessed by clicking this FORMS link or by clicking Forms in the navigation bar at the top of the page. If you would like to complete your forms here in the office please arrive 20 - 30 minutes prior to your appointment. Be prepared to present your identification and insurance card when you check in.
Immediate Care: Our goal is to treat urgent medical problems within 24 hours. If you need immediate medical attention during regular office hours, please call our office. Some appointments are set aside for emergencies.
Please remember that your insurance may require a doctor’s referral for emergency care that is not life-threatening
Payment & Insurance: Payments or co-payments are required at the time of service. We accept Visa & MasterCard. We accept most insurance plans. For your convenience, we will file your claim. Please bring your insurance care when you visit the office so we can file your claim properly.
- Ask questions, share your feelings and take an active part in your care
- Be honest about your history, symptoms and other important information, including any changes in your health and well-being
- Take all your medicine as directed
- Inform us whenever there is a problem with the medication you are taking
- Make healthy decisions about your daily habits and lifestyle
- Keep your scheduled appointments or reschedule in advance whenever possible
- Call our office first with your health concerns
- Be sure you leave our office with a clear understanding of our expectations, treatment goals and future plans
- Request interpretation services, if needed
As our patient,
your responsibilities are:
Bring to your appointments
- Photo identification, insurance card and payment
- Any prescribed medication, in original containers (include over the counter, herbal therapies and vitamins) or a detailed list
- Any records or logs of self-care health management (for example, blood pressure or blood glucose management)
- List of your health questions
- List of other providers you have visited (including their names, addresses, phone nubers and reason you visited them)
Whole Person Orientation
- Listen to your feelings and qustions which will help us make decisions about your care
- Set health care goals with your input
- Explain diseases, treatments and results in an easy-to-understand way
- Give you clear directions about medicine and other treatments
- End every visit making sure you have clear instructions about expectations, treatment goals and future plans
- Provide you with a copy of your care plan and/or a clinical summary after each visit
- Share tools and education to help you manage your care at home
As your personal provider,
our responsibilities are:
Access
- Provide same day appointments whenever possible
- Provide instructions on how to meet your health care needs when our office is not open, providing you with 24 hours access to medical care
- Provide electronic access to your care team and healthcare information through a secure patient portal
Coordination of Care
- Send you to trusted specialists, if needed
- Provide key clinical information about your to facilities (such as hospitals and emergency rooms) and other providers (specialists, labs, x-ray and imaging, physical therapy, home health, medical equipment, etc) who help take care of you
- Request records from other providers to reduce duplicate services and costs and to avoid medical errors or missing critical information
- We are members of the Kansas Health Information Network (KHIN), a secure electronic network that streamlines healthcare information for health professionals
Quality Care & Safety
- Provide evidence-based care and support you in self-management of your care
- Use technology such as electronic medical records and share records to help prevent medical errors and make sure that we are always on the same page
- Keep your electronic health record updated with comprehensive information regarding your health care, such as medical history, medications and test results
